Know anyone who died in 2021 or 2022?

If so, I need your help. Please fill out a short survey for me. It is really important. It could end all debate. *ANY* cause of death qualifies for the survey!

I think this may be my most compelling survey ever.
If you know anyone who died in 2021 or 2022 for any reason, I need you to fill out my survey. This includes ANY reason… hit by a truck, suicide, whatever. It does not matter.
If you know more than one person, just report the person you know the best.
There just ONLY five required fields so only fill it in if you know (or can ESTIMATE) ALL five items about the deceased:

1. age
   
2. date died
   
3. Vax status: Unvaxed, 1 dose, ….
   
4. If vaccinated, we need the date first vaxxed
   
5. your relationship with the deceased
   

That’s it. All the rest is optional.

Note: Please be sure you get the year right when filling in the date. That’s VERY important.

I’ve been working with Joel Smalley on this. We are both very excited about this. You will be absolutely amazed with what we can do with that data.
